Clinic care team member sign up/registration - without existing Tidepool account

Description

A Clinic Member will be registered by a process initiated by Clinic Administrator to allow them to begin using Tidepool.

As a reference and separate from the scope of this ticket:

  • Backend ticket covers the invitation and acceptance mechanisms for email sent to clinician user.

  • After user taps their invitation, the clinican user will be prompted to set a username (required) and password (required) to set up their account. [ ]

  • System shall enable a clinic administrator to register a clinic care team member

  • To register a new clinic care team member, the admin will need to enter the following information and assign roles and permissions accordingly.
